"Accident"
An undesired event that results in
Harm to people
Damage to property
or
Loss to process

The definition is quite complex. Try here:

It is basically any work-related accident or ill-health resulting in death or one or more shifts' absence from or restriction from normal work or requiring specified medical treatments. The day of the accident does not count.
The OSHA site does have a fairly good FAQ on this as it's quite detailed.

Simple definition of an OSHA Recordable is
i. Death
ii. Days away from work
iii. Restricted work or transfer to another job
iv. Medical treatment beyond first aid
v. Loss of consciousness
vi. A significant injury or illness diagnosed by a physician or other licensed health care
professional.
Don't forget, here in the US, OSHA does not have responsibility for accidents to non-employees, like the HSE does in the UK.
Additionally there is not socialized medicine so each state has WC insurance and will have incident/accident/claim definitions also.
