I'm reviewing our present fire incident report form. The current fields are as follows:
Date Of Incident
Time
Premises Name
Address
Where Did The Fire Start?
To Where Did The Fire Or Smoke Spread?
Description Of Incident (Include Details Of Witnesses)
Numbers Of People In The Building: - Workers: Non-Workers:
Time Fire Brigade Called
Time Fire Brigade Arrived
Time Fire Brigade Left
Did Brigade Fight The Fire?
Are Fire Brigade Preparing Report?
Name of Fire Officer
Was Premises Evacuated? Time Taken:
Time Premises Re-Occupied
Who Used The Fire Equipment?
Number & Type Of Fire Equipment Used By Non-Fire Brigade Person(s):
Description of Any Damage Caused (Including Smoke Damage)
Description of Any Injuries: (Complete accident reports if any injuries are reportable to Safety Team)
Have Photos Been Taken? Taken By:
Additional Comments: (Did Fire Procedures/Warning Systems/Fire Doors Operate Correctly?)
Did the fire cause suspension of normal work for over 24 hours?
If Yes, Date/Time Telephone Notification Made To Enforcing Authority:
Action Already Taken To Avoid Recurrence Of Incident: By Whom?

Ron, this seems quite comprehensive as a "what happened" form. What seems to be missing is the part after - the investigation into the (root) cause(s) of the fire, actions / improvements required etc. Possibly you would have a diferent form however it is probably best on one form, keeping it all together. I would also reccomend having this part of the form circulated and signed off by the great and good (Manager, Engineer, Safety Person etc) to give the actions credibility.

Hi,
Post fire, I would suggest that you need to review the significant findings of your fire risk assessment, and then take the appropriate action.
Your existing fire risk assessment should take cognisance of the previous history of fires, with a cross reference to both documents.

How many heads activated?
Length of business interpution/sales/production whatever (could be longer than evacuation)?
Cause, Deliberate or accidental, If deliberate provide copy of police report? Name and contact details of inevtigating office?
