Has anyone answered the question HAZOP study, Show in tabular form how deviations by a hazop study could be used to predict the possible causes and consequences of failure to transfer petrol successfully from an underground tank to the nozzle of a petrol pump on the forecourt of filling stations, and identify actions that might typically be required to minimize the likelehood of such a failure

Hi, this is a question that has been previously raised in the study support forum so what you want may already be posted there.
You can also find examples of what you need in Kletz's books on hazop and hazan and its also worth you looking on the institute of chemical engineers website.

HAZOPs are easy - quite boring after a while
Just define your key words and parameters and identify the possible deviations and evaluate the consequences/remdial actions
e.g. PRESSURE HIGH - vessel could burst, excess flow rate
PRESSURE LOW - no product flow, feed valve shut etc etc
