Posted By Giles Davis Hi all, I am about to write a new accident investigation procedure for our company based on the HSE's HSG245 'Investing Accidents and Incidents' using their 27 part 'questionnaire'. Has anyone already done this or got a format based on this they could give me in order to help me from re-inventing the wheel so to speak? Thanks and regards Giles

Posted By Giles Davis Charlie, I'm not really sure. The accident investigation format I'm using at the moment was provided by a colleague 5 years ago when he came to work for us. As I didn't design it, whenever I use it I never quite feel that I've completed it as it was supposed to be. We are an SME with 41 employees so only have a few accidents a year that are usally manual handling or chemical irritation ones. The only 'training' I've had in accident investigation goes back to NEBOSH Gen. Cert. I'm qualified to Dip2 now but have to hold up my hand and accept that accident investigation is not one of my strong points, hence the request. I can construct what I want really using HSG245 but it will take a few hours on MS Word and I was trying to save a bit of time. If you think your format will help me then I'll be very grateful for a copy. Regards Giles
