Have a look at RoSPA Accident Investigation Training Course at; http://www.rospa.com/occ...cident_investigation.pdf
For PTW training you could try Empower Training near Nottingham as they do HV and LV engineeing training including some safety document issuing so they may be able to help - I have used them and they are good. Look at http://www.empower-train...ng/engineering/index.htm

The problem with PTW training is that ever workplace has it's own system, hence generic training is of little use. You generally need to train "in house". The HSE guidance on PTW does a leaflet INDG 98 http://www.ihsti.com/tem...-OHSIS2051900453117.pdf.

This is not much use but the back page has some very useful links listed, especially

Guidance on permit-to-work systems in the petroleum industry HSE Books 1997 ISBN 0 7176 1281 3

The safe isolation of plant and equipment HSE Books 1997 ISBN 0 7176 0871 9

Have a look at the two IOSH Accident Investigation Courses on the IOSH web site. The four day course is designed for those with responsibility to investigate accidents themselves where as the two day course is aimed more at managers and supervisors. Otherwise contact ian.langston@wanadoo.fr to speak to the tutor

Tip

I am not sure what the work situation is. You may want to try coaching and one-on-one support to work with you through your own systems for injury investigation and permits.

The reason for this is that people can go on training courses, but if their own system isn't watertight the training can be ineffective.

Please drop me an email direct if you wish to know more.

Kind regards

Ian

Tip,

Have you tried the University of Bolton, they do an accident investigation course. Chris Minta is the contact there.

Regards.
