You walk past a building site. Tilers on the roof. No scaffold. Forklift with cage on, cage up, no driver. Rope from lift hydraulics to top of man-cage ?
Hole in the ground. No barriers.
In the factory. Bad light. No ventilation when the doors are shut. 380 volt expension leads. Heaters that recirculate the air, no ventilation remember !
Lathe left running, chuck key in the chuck still. Panic stop button removed from switch unit. chuck guard up, microswitch held in "on" by piece of chalk.
Guillotine: guard levered up and blocked with metal.
Some days get really bad......

Walked into a NHS hospital workshop...... the workshop looked like a bomb had hit it tools and equipment everywhere floors, kitchen area, toilet !!.
No training records (or training ever given) to operate grinders, saws, welders etc - no pat testing - no first aid kit and missing fire extinguisher... it was ok though the fire reel hose was still available ish. The hose head was missing and the hose was used to wash the boiler room floors / cars and top up the cooling tanks.
Trainee engineer was left on his own working inside a large gas boiler which was still fairly hot !(confined space) and I had to let him out of the boiler as the end had closed on him and although not locked it was to heavy to open on his own....lucky I was there, even luckier the mortuary was next door although not required on this occation.
Decided after talking to the hospital manager and after other worse situations to leave the NHS - VERY Quickly.

When I first started in H&S and did a ton of work to get procedures up to date I decided to leave it a couple of months and do audits. You know the kind of thing make sure records were being kept, RIDDOR's completed, training up to date.
I was however frustrated to find that our managers did not hang on my every word and implement procedures as fast as I could write them and I would fly into a rage,stomp about the building pontificating and spitting feathers.
Then a wise woman told me. 'Jay you know these guys keep you in a job'
Wise words indeed.
