Disabled service user needs two people's assistance while being washed in the morning. One to actually wash and the other simply to stand at the other side of the bed and act as a back-stop as he has a tendency to roll. On the morning in question one carer, a permanent member of staff, responds to a call buzzer, leaving another member of staff, agency, to deal with IP on her own. IP rolls away from agency worker on to floor, fracture neck of femur. RAs in place, care plan detailed and comprehensive, culture of compliance, member of staff disciplined, enforcers take no action,

thanks for that John . I really wanted more global buisness risk related cases which encompass loss of market share - reduction in service reputation and subsequent funding type things.

Kindest regards Bob

Oh, OK,

How about Anchor with the disastrous TV programme a couple of years ago? Can't find a link on the net, but one of the investigative news programmes put a mole into one of their services, the resulting TV did some damage to their reputation,
