Just wondered if anyone had a simple but effective near miss form they could send me. The current system on site is poor to say the least, 5 recorded near misses in 2005. I would like something thats catchy but easily understandable to the masses, and get away from the plain old forms that everyone perceives as an extra bureaucratic system.

However instead of just redesigning the form, have you thought of relaunching the entire near miss scheme. I was in the same situation in Jan 06, very few near miss reports coming through, since then we have highlighted the importance of near miss reporting, through various posters, toolbox talks and general information and we have had more reports in this year to date than for the previous 2 years combined.
It is equally important for employees to see that what they are reporting is being investigated and resolved asap.

Posted By SAF I'm in the middle of relaunching our Incident Reporting systems. Would be very interested in seeing what others have implemented. The simpler and user friendly the better.

Posted By Incolumitas We do not make a distinction. Everything that, in simplest terms, is not as it should be is reported on the same, simple, incident report form.
Rather than have a report form which asks for lots of detailed information, we ask for date, time and place; a brief description and the names of those involved and the capacity in which they were involved. This is just 1 side of a sheet of A4
The simplicity encourages quick reporting to those charged with investigation, who in turn are properly trained to acquire the necessary detailed information.
This also elevates near misses, hazards seen etc to the same level as serious accidents - they are all safety incidents and all need to be looked into on the basis of POTENTIAL severity rather than severity of outcome.

Posted By Ron Hunter There are numerous Forms available via Google, the issue is making the system work. I read somewhere lately (can't remember where)of an Organisation which had set up a system of a charitable donation for every near miss report received. This seemd to spark the imagination of the workforce and the Employer was delighted with the results. Employees should be involved in chosing charities.

Posted By paul eastell Jon, Don't know the size of your orginisation, but, we, this year moved to a hazard near miss database. All teamleaders, shift managers have access to the computer system. We will soon also have an independant terminal but this has not stopped the progress as we have had 160 reports YTD. Database = actions not completed, hazard remains open and only the originator can approve the report and close it.....masive improvement to paper system. This still requires person (me) to chase it up but has made an impact on our safety performance. Glad to pass details if needed.
