Good morning,
Would anyone be kind enough to recommend either a publication or web site, where I can develop my understanding of the multi-causality approach to accident investigation,(‘immediate’, ‘intermediate’ (sub) AND ‘root’ causes)any assistance would be much appreciated.

Try the US Chemical Safety Board. Look at some of their reports.
The ongoing investigations into the Buncefield and Texas City explosions are worth following. Each has a website.

The Texas City website set up by BP appears to now be unavailable to the public - no reason given.
The Australian Transport Safety Bureau is also useful -excellent accident reports that go beyond immediate causes.

The IOSH book on Risk Management by Tony Boyle covers this area in Chapter 20. IOSH also runs a short CPD course led by Tony Boyle and another by Ian Langston, which uses a good generic model for root cause investigations. There is draft IOSH guidance which also outlines this area, though not yet quite ready for publication - but both these people, and others, have contributed.
