Does anybody have a pro-forma risk assessment for assessing the fitness of staff working in the rail industry if for exceptional reasons they are required to exceed the industry working time parameters?

I am not sure where you are coming from? The fitness and working hours within the rail industry is controlled in part by regulation and in part by contractual requirements. Either way, they are obligatory and there are no 'exceptional reasons' that I know of for 'risk assessing' them away.
Please explain further if I have misunderstood your question

Yes. Railway Group Standard GH/RT4004 refers to these circumstances.

yes i have one and has been approved via linkup audit as satisfactory. email me and I will send one to you. Also there are times when you can exceed parameters but these are exceptional and not to be used for planned exceedencies.
