We Have been running a behavioral safety program at our company for about 18 months. Observations, safety tours & training for staff.
I would now like to conduct a survey to assess impact & cultural change the program has made.
What I am wanting to know is, if any of you out there have done anything similar & could you stear me in the direction of some kind of standard template for this kind of survey?

A useful survey in this area depends on the reliability and validity of responses. These in turn are influenced by the quality of the questions.
As your heading refers to 'climate' and your question talks about 'culture', it appears that some refinement in your approach is advisable at this state. An inadequately designed survey risks creating barriers to implementing your findings.
The leading writers on behavioural safety (e.g. Scott Geller, Tom McSween, Dan Petersen, Tom Krause) each provide reliable guidance on how to proceed with a fair level of control of the risks involved in such a survey.
