Posted By Al Beevers Gary, What do you need the permits for? Permits are used for the most part for high risk work, where there are residual risks that can't be worked out of the safe system of work - as you mention confined space entry. To use Permits safely, the issuer and the acceptor of the permit must both be competent to do so, and a full risk assessment of the yask being carried out needs to be done. All equipment must be checked, and environmental monitoring, isolation of plant (all by competent people) needs to be done. The permit sheet that you write out is only the final step in the process of ensuring all the above is in place. There must also be clear rules for the monitoring and suspension of permits, and provisions for emergency situations - Do you abandon the work if the fire alarm goes off? Permit to work sheets should be designed for the task/industry/employer and even site. I doubt that anything you could down load from the internet would be viewed as being sufficient for safe use. If you need to read up on using permits, the HSE have a leaflet HSG250 which is very good. Al.
