Posted By SeanThompson I'm putting together an accident reporting database rather than using the accident book, what additional fields to people have? i will be using the accident book fields but i am wondering what else people ask for? does anyone have any copies or examples of the things they use?

You should check this out with the Department for Work and Pensions as the BI510 accident book has legal status. They are quite happy for alternative records to be used but you should get their permission first,

Perhaps our forms may differ but this is what we include...
Category i.e Incident or Near Miss Date Time Site i.e building location Name and job title Any witnesses No of hours days lost due to incident Type of incident e.g. Manual Handling, Fire etc Whether person was seen by i.e. First aider/doctor etc and the action taken A description of the event (Detail) Whether Riddor Reportable Cause of incident and whether covered by a procedure Action taken to reduce risk/remedy effects Any further action to consider Any feedback given by Manager. Space to sign and date.

You can for H&S purposes, but the BI510 is used for Industrial Injuries Claims under a set of Socal Security regs, and strictly speaking you need DWP approval to use another format. It's a very simple and fairly informal process; I rang them, was given a name to email, emailed the form and was sent an email back with permission. It took a few days,
