The HSE have free online resources such as www.hse.gov.uk/vibration/hav/readyreckoner.htm & ....vibrationcalc.htm to take you through the stages of a HAVSs R/A.The outcomes/further action required is determined by the scores obtained.
I suggest that a Risk Assessment process can be 'built' around this free toolkit and information.
Have you got your Occ. Health Protocols sorted out for A(8) above 2.5m/s/s?
By the way - a big 'well done'& thankyou to the (recently) much lambasted HSE for providing all these free resources!

I think you have mis-understood my question.

I have everything in place, the only thing I'm looking for is a blank HAV risk assessment form/proforma if anyone has one I can look at.

Ah! My answer then is to use a 'standard' Risk Assessment Form with a copy of the HSE HAvs Calculator for the Task(s) or process(es) attached.It is the score from the calculator which will determine what controls you will establish and what further action you will take.
I don't see HAVs as requiring the invention of another type of risk assessment form.
