Also does anyone have any risk assessment audit process templates/paperwork to share?

Welshboy, from another Welsh boy, we need to know what area you are working in to be specific about a checklist. In Industry generally the sort of issues to cover will be the same as everywhere else with differing emphasis. Lighting, Noise, Temperature Ventilation Extraction, Housekeeping and waste, Traffic routes and passageways (floor condition), Emergency Exits, Falls and falling objects, Electrical safety, Hazardous substances, Handling those substances, PPE, Machine safety, Tools and equipment, Lifting and handling, Ergonomics (work and job design), First Aid facilities and Safety systems.

There are a few available on the internet which are pretty good but the problem I find is that using an audit sheet with all of the above items on it makes for a very long audit or a very superficial audit. It might be better to do an overall housekeeping audit and use all the other subjects as individual items over a year.
