Posted By Jason McQueen Im looking to replace/redesign our internal monthly inspection forms and was wondering if anyone could send me theirs to get ideas of layout and content (obviously I'll send anyone mine if they want to trade).
Our current form comprises of a series of questions based on a tick box form. My concern (or rather one of them) is that it's too prescriptive and that people are only looking for items that are listed and choose to ignore anything else.

I am currently doing a similar exercise.
We are using 'themed' inspections targeted at out current high risk factors, or potential risk factors where the person completes observations. I am finding we are getting better quality observations.
We direct an individual to look into specific work areas or risk areas and concentrate their inspection on that issue.
The ticking of boxes is generally on Mandatory requirements and can often be captured separately.
Our forms are not very user friendly and the tendency is to just tick box after box. Themed inspections make more use of the comments box.
