Accident and near miss recording and presentation of data

I have been asked to set up a accident and near miss recording system. The directors in our company want to identify trends, accident types/injuries and troubled spots in order to take remedial action. We have 6 different locations. We are a transport and engineering firm.
I was wondering could anyone give me advice on:
Encouraging management to report near misses
Presenting my findings to directors i.e. graphs
Benefits to the company of near miss reporting
Near miss reporting forms
Any help would be appreciated.

I think you meant encourage staff to report near misses and encourage management to take them seriously.
Near misses should be treated as a 'free lesson'. Unfortunately capturing near misses is not so easy - the Holy Grail. However, and depending on the number of incidents, you could categorise them from say 1-3, then re-examine them for their potential loss. This data could be illustrated by using a simple bar or pie chart.
Once re-examined, use the significant data to provide measures to prevent re-occurrence and close the loop by ensuring all staff are forewarned with a safety alert, revise procedures etc.
Ultimately you get out of it what you put in.
