Hi
Has anyone got a copy of this accident investigation document they could email me at all? If not then would someone be willing to share their template for accident investigations. I have one to do next week and as this is more than likely going all the way, I wanted something a little more robust than my usual one.
Any help would be greatly appreciated

The document your looking for is protected by copyright law and if you read the small print at the foot of these posts you will see that IOSH discourage infringment.
