Posted By Phillipe Hi Has anyone got a copy of this accident investigation document they could email me at all? If not then would someone be willing to share their template for accident investigations. I have one to do next week and as this is more than likely going all the way, I wanted something a little more robust than my usual one. Any help would be greatly appreciated Phil

I dareaay you will be forgiven for this oversight. HSG245) are available from HSE Books, PO Box 1999, Sudbury, Suffolk CO10 2WA (Tel: 01787-881165/Fax: 01787-313995). Priced publications are also available from good booksellers. ISBN 0 7176 2827 2, price £9.50. or if your organisation subscribes to a sevice such as technical indices you may be able to access a copy.
