We are revising our PPE Issue Receipt. As retaining copies of each PPE Issue Receipt is becoming a nightmare we are suggesting that we record the first issue and include on the receipt and in the Employee Handbook "IT IS THE RESPONSIBILITY OF THE EMPLOYEE TO MAINTAIN AND REPLACE PPE AS REQUIRED. PPE MUST NOT BE LENT TO OTHER PERSONS OR ALLOWED OUT OF THEIR CONTROL. OLD PPE AND BRANDED WORKWEAR MUST BE DESTROYED TO PREVENT UNAUTHORISED OR UNINTENTIONAL USE". We'll monitor use through inspection and over-consumption (i.e. car booters / theft) through reviewing invoices. Any one see any problems with this? Is there are a legal requirement for a receipt per issue.

There isn't a legal requirement for recording any issuing of PPE. Recording the issue is to ensure you have records that individuals are in possession of the required PPE should they claim they were not, particularly following any incident. Ensure the general PPE training you provide includes issues like reporting defects, how to obtain replacements, and stopping work if PPE becomes damaged, and record that training.

You do not, IMHOP, have to prove each issue. It is better to have in place a provable system which ensures issue as required. Most people can make a pair of safety shoes last a couple of years. Others (maintenance) need a couple of pairs each year.
Just an "as required" or "on demand" system will do you.
Some places I go to, automotive press shops for example, will ask employees to count how many pieces they handle before the glove wears through. Then they will ensure replacement beforehand.
No body ever signs for anything. It's a foreman level type thing.

Hi Darren, I do go down the route of getting all boots, hi viz vests and coats and an industry specific tool signed for on issue. The form details reason for issue, any specific instructions for use, checking to ensure PPE is fit for use, storage & cleaning of equipment & why the individual is being issued with it. It clearly states that defects in the equipment must be reported to their line manager and that wilful damage will not be tolerated. This in addition to managers completing monthly checks.
It is logged on a database so that people who request a pair of new boots every other month are identified immediately cutting out the "car boot - excuse the pun" problem. As I look after 500 staff, the consequences of failing to keep a close eye on duplicate issue can lead to the need to order vast quantities of PPE... Sometimes wonder if we employ centipedes!! :)
