Has anyone come across incidences of employees collapsing as a result of starting a course of nicotine patches?

We had a temporary worker collapse a couple of weeks ago and the hospital advised that this was the cause.

Having spoken to a few medical friends of mine I am a bit dubious to say the least!

There appear to be a few 'urban myths' around...

People tend to start off with the most powerful patches they can obtain. They want to stop the habit quick and be sure that the patches will work.

Remember when you were about thirteen and your first fag gave you that "buzz" ? Well, this is what can happen with patches. More of a boing than a buzz.

People should start with lowish level patches and, if they don't kill the craving, move up slowly.

It's all a matter of dose.

Medical advice should be obtained before choosing an appropriate dose. And complete honesty on how many a day you get through. "About 10" when it is actually two packs will not be helpful. To you or to your doctor.

We had one such incidence in our place. The indiviidual concerned was actually in hospital for 24 hours for observation. It also turned out that away from the working area he was "topping" up with a fly fag.

I'm currently on the full strength patches again for the third time in two years (they work but I'm weak) - note this of course does not make me an expert.

While I have never collapsed, nor has anyone I know who has used them, they do make me feel a bit light headed in the first couple of days.

Another side effect is the instructions for the clear ones say wear them for 24 hours, but when I did this, I had difficulties sleeping and extreme nightmares.

I would recommend that you ask the person who collapsed if they could bring in the contra indications leaflet out of interest to see if this is a noted side effect. As this occurennce is obviously potentially and significantly hazardous I would bring this example to the attention of the manufacturers.

The hospital must have got their opinions from somewhere, can you go back and ask as a general enquiry? I would also refer the person to go back and report this to their GP who can prescribe a suitable alternative. The GP and the employer can not rule out other explanations for the collapse.
