The investigation into the Cumbria rail crash continues, but presumably people have heard and read the comments in today's meeja that the police investigation is being complicated by poor record keeping, lack of management control and so on. What do we think? Is this just spin, or has the rail industry still got so much work to do after Potter's Bar and Hadfield? And if so, why?

My experience is that if people are not made aware of their responsibilities then they will not undertake them with the same vigour and urgency as those that would if they were aware. This must come down to poor mgt of health and safety behind the scenes. Paperwork, nobody likes doing it. I would imagine a proper audit trail would highlight this as a shortcoming in many industries, not just rail.
Take a weekly fire alarm test. These should be recorded for every weekly test and signed off as serviceable or not. If not serviceable then a call made to correct the problem. I do not think that is an onerous task for any human being, but you would be surprised for how many people it is over and above them...sometimes I despair to the point of leaving this industry.

Posted By Robert K Lewis The real issue behind all of this is a distinct lack of competence management by the organisation. When one considers that such a system is mandatory for signaling work one will continue to ask questions concerning the strategic management of the rail network.
I am addressing this problem at the Midland Branch conference on May 8th, see the news releases on the IOSH home page - Competence Design and Management. Whilst the general setting is construction there are lessons to be learned for all.
