I have tried to cut and paste a form we use at our warehouse, not sure if it looks ok, but hopefully you will find it of some use.

Hello David,
Refer to the operators handbook, in there you should find a comprehensive list of daily, weekly and monthly check / service items which should then be quite easy to put in to a check list format.

FORK TRUCKS DAILY CHECKS
****** ENGINEERING LTD.
Turn key on, start engine, drive off.
If when key is turned engine fails to start, get on another truck.
If truck breaks while driving, walk away and deny it.
If truck overturns in yard, give driver three days off with pay and don't RIDDOR it.

The check-list offered by Clare is OK but the problem is actually getting them to do the checks rather than just checking the boxes.
Best system I know is for all trucks to be parked up at the end of shift outside the foreman's office. Incoming shift then do the checks in concert under the foreman's eye.
Takes five minutes.
