Today on my construction site was near miss:

on roof of one blocks crane was passing metal bar, it was size about 2 metres and weight around 70 kg
- it was tied in the middle but during the operation it tie moved to one side and it only stopped because metal plate was fitted on the end,
all this happened over one of employee,
possible results:
fatality, major injury
What actions should be taken?
My recommendations are:
a) Tool Box Talk about safe methods of lifting
b) Training with recognised company
C) Remove banksman from site because it was today his first day and in my opinion this showed his competence...
He is employed through agency.
What is your opinion?Correct me if i am wrong.

In order to determine what to do, you need to understand why the incident happened.

1. Was the correct lifting equipment used? If not, why?

2. Was correct equipment used to attach the load? Of not, why?

3. Was correct method used to attach the load? If not, why?

Your solutions do not seem to address any root causes.

Training on its own is rarely a complete solution. Did people lack skills, or were they not allowed to use them because of lack of equipment or management pressure.

You may be correct in saying the banks man is incompetent, but if that is the case how did he get hired in the first place?

Near misses provide an excellent learning opportunity. The great thing is that no one has got hurt, and so people will have less emotion about the incident. I'd dig much deeper and you may find out things that will lead to massive safety improvements.
