Hi Lewes,
I am also putting together a similar PPT, aimed mainly at disc cutters (Stihl saws) on construction sites (see my post on "Disc cutter accidents"). There is bound to be a lot of information common to both, so if you want to pool resources, let me know.

PS I forgot to add I am familiar with a couple of incidents were loose clothing got entangled such as the strings from hoodies or the string from the hood of wet weather gear as the IP bent over the grinder.
