There are probably not many who wil have a blank pro forma to offer you becasuse in very simplified terms it is a schedule of events that you would produce for whatever task requires it, and would become part of your overall safe system of work.

Unless a client/PC/other etc stipulate, then a simple word document and you can dress it up or down to whatever is right for the occasion; start at:

ARRIVAL
ACTUAL TASK/S
DEPARTURE

Just create a detailed submission that shows warts and all what you intend to do, who is doing it, what safety features you wil use, equipment detailing, product detailing, any licences (MEWPS etc) training, supervision, signing in/out, any permits to work required, liason with client/customer etc, any required vehicle/pedestrian segregation, signage, changes to any routes for pedestrians/vehicles, special arrangements, lighting, access, keys, etc etc (basics only, included here for illustrative purposes) until you have finished; and depart.

As long as it is detailed and accurate it need not be overly fancy. Out of 200 contractors that do work for my Group I think perhaps only a couple use excel in place of Word, as long as it works then that is all that is usually required.

All the best

The following is assessment criteria used by some clients during pre-qualification. It should give you a steer on what is expected.

Assessment Criteria

Method statements should identify the sources of harm for the work activity and set down the operational safety controls needed to prevent harm

Typical information required should include but not be limited to the following: -

* Description and the location of the work,

* Roles and responsibilities (including supervision arrangements),

* Sequence of work activities (including specific site rules and restrictions),

* Resources

* Emergency response arrangements, and

* Competence level for workers and their managers (names provided)

Best wishes
Ciaran
