Does this need to be raised as a near miss report?

A convoy took place on 13th June, 2007 to move six loads from one rig location to another and part of the itinerary was to go through a centre process facility route, once we reached the centre one truck (leading the convoy) hit a street lamp with the corner of the skid he was loading, nothing has been noticed at the time of the incident, until a QHSE advisor, driving a land cruiser, over passed our vehicle, and informed us. Does this need to be raised as a near miss report? Please advise.

I would say yes, depending on your company incident reporting procedures. You will probably have some in-house guidance available which gives the criteria for reporting incidents depending on potential - this one looks like a high potential to me, bearing in mind the risks associated with rig moves. If it happened again it may not be a streetlamp the skid hits.

Sorry - just read your post again and realise it wasn't actually a rig move but a convoy moving equipment from one rig location to another. Doesn't change my view though - should still be reported as a near-miss.

Posted By Robert K Lewis Seems like a near hit to me. Near misses refer in essence to things that could have happened and not things that did. The fact that you hit a lampost without knowing suggests that there was something wrong with your systems for banking these vehicles when they are on the move.
For me near misses are things that raise the risk level in an operation rather than an incident that could have caused injury.
