I know I can look on sites, but i'm a bit short of time.

Does anyone know of a prosecution case that I can use as an example in a presentation I have to do to Managers that's basically in the following format:

Name of company & how much they were prosecuted and what the injury was.

A few bullet points on what was the chain of events that lead to the accident, i.e. no risk assessment, poor maintenance, untrained staff etc.

If possible an example in road work or working with plant?

Easy peasy - HSE prosecutions are listed on the Prosecutions Database. Go to HSE home page and follow links to Enforcement Action.

There is corresponding database for Notices.

Only problem is that these do not include info on Local Authority enforcement

There is a topical case in this month's SHP (p11) regarding the death of a piling rig operator and subsequent prosecution. The company were fined £75,000 with £34,425 costs for breaching section 2(1) of HSWA 1974. Root causes were a failure to provide a SSoW and underlying cause was failure to formally assess the risks.

How about the Photo-Me case - fined £100,000 plus £30,000 costs, for three cases of contact dermatitis to photographic chemicals, largely because they had no proper management system in place?

Two Irish Cases which may be of benefit can be downloaded from the Health & Safety Authority Web site at http://www.hsa.ie/files/...thSafetyMattersmar07.pdf

One resulted in €50,000 Fine for Safety Breach
by a Plant Hire Company following a prosecution for a serious incident, which resulted in two electrical explosions.

The second resulted in €250,000 and suspended jail terms of 2 and 3 years respectively have been imposed on two companies and two individuals as breaches of health and safety legislation resulted in the death of a worker.
