Should I include a reported case of fatigue (wrists aching) as an injury/accident ?

Is there an identifiable incident or task at work that has caused the aching wrists? I would be looking for the person to get their GP's opinion and maybe then involve an OHT but I would be very wary of accepting any blame without getting some facts first.

If the injury resulted in more than 3 days off work, yes - PROVIDED and to the extent that you also conduct, or arrange conduct of a competent ergonomic assessment of root causes of the aches.

You can of course always report things but I would caution otherwise.

RIDDOR relates to an "injury" and is clearly aimed at a specific event in time and place - e.g cutting your finger. "Aching wrists" is not an injury and as far as I can see it is entirely recoverable. You wouldn't send a report to the HSE just because an employee complained they'd worked hard and were tired would you?

If however this was diagnosed as a notifiable disease such as tenosynovitis by a doctor and you received a (legible) doctor's note to this effect such a condition might be notifiable. You might still accept such a condition and act without formal correspondence from a doctor but your are not obliged to report it.

My own maxim is only report what you have to!
