Can anybody help please

I have been asked to launch a climate survey questionnaire during our health and safety awareness week.

I have been on various websites and have spoken directly to the H.S.E. and nobody seems to be able to either provide or sell me a copy of an actual climate questionniare sheet / package.

Can anybody advise me where I can obtain a copy or purchase a questionnaire.

They are re-writing it and have taken it off the shelf for the time being.
