We have operators who are exposed to noise and vibration but our assessment considers it a low risk taking into account working procedures, PPE, exposure times, and the level of noise and vibration.
We acknowledge that health surveillance is required but in what form? Is is mandatory to have medical assessment by occupational nurse?
or can be based on personal feedback from medical questionnaire?
Practical and reasonable replies please.

You should consult your OH provider, but in the first instance most OH providers will issue a questionnaire to any employee using vibrating equipment to see if there is any signs of any problems.
You should also be carrying out a risk assessment - ha and isn't this a hot potato - HSE has provided a useful tool on their website http://www.hse.gov.uk/vibration/hav/index.htm
along with a lot of useful information.
We found a reputable consultant who helped us but I am of the opinion it is all a bit difficult for the average H&S person to do/understand/deal with.
