Hi All,
How common a problem is the incidence of Minibus removable seating(wheelchairs),having been replaced, then becoming detached in use?
We have trained & re-trained staff but are still getting some incidents. Please advise how common this is & what actions you have taken?

We were having the same problem at my work, despite training and retraining staff they didn't do it frequently enough to remain proficient at it. We now have a policy that when seats are reinstalled they are checked for correct installation by myself. The consequences of them not being installed properly are very serious and since we have adopted this approach we haven't had another incident.
