Ladies/Gents, I am trying to make it easier for my site (construction personnel) to raise near miss/H&S concerns through a simple postcard system. whilst I can produce one myself I don't want to reinvent the wheel.

so!! can anyone point me in the direction of a tried and tested card?

your assistance is appreciated.

we have a very good near miss/hazard reporting system. What I am trying to do is place postboxes around the construction site (tis quite large) with some simple to complete postcards that contractors can fill out in a minute and raise a concern, the reason for this is they can do it anonymously (if they wish) and not feel under pressure to raise concerns

Sorry to miss understand you but I thought you were after a complete system???.

The cards I use are very simple the only require the location of the concern and a brief description. there is no need to put you name to this card unless you wish to.

These can then be placed in boxes provides at places such as the rest rooms, changing rooms, and even in the toilet areas, the boxes are checked each day and any concerns are followed up ASAP ensuring that people see that these concerns are being taken on-board. Then the outcome of the concern is put on the notice boards. Each month a list of raised concerns is also place on the notice along with how many have been resolved.
