Are you specifically after a word based PTW per se for a variety of tasks (hot/CS/Roof/BES/EX etc)? or a word document specifically designed to assess the value of PTW from others?

If you follow this link you will find HSE's Hazardous Installation Division's Inspection Manual for Land based operations. This contains a whole series of question sets that can be used to assess various 'risk control systems'.

Page 106 is the start of the section for looking at Permit to Work Systems. I used this question set for a PTW audit i had to do and thought it was very comprehensive. It also follows the HSG 65 format.

This is a good example of some of the very useful but very well hidden info on the HSE website. This is not to be found by browsing under the chemicals section but the freedom of information section.

Unfortunately it is pdf. However you can easily use the text select function in acrobat reader and then copy and paste into word. A few minutes of reformating will give you a useable document.
