Try the Nottinghamshire Fire and Rescue Service site. Click on "Protect my Business" (bottom left corner), then click on "Risk Assessments". Here you will find a blank and also a worked assessment that you can download.
I suspect this is available on all county fire service websites. You may well be able to find one for your local Service.
