Has anyone ever investigated a property damage accident were a industrial roller shutter fell off its rails?

I am sure you have been there with other investigations were the manufacturer tries to blame the installers and vice versa. all I want is the truth to enable me to put measures in place to prevent a recurrence. I have tried searching for other similar incidents to no avail, I have tried to find an FMEA on the subject and ditto. in a nutshell the roller wheels seem to have come out of the track whilst it was in the up position and fell off, fortunately this was during silent hours and nobody was injured, however the potential is quite serious.

any ideas welcome. I know you cant investigate and I don't want you to speculate I am more interested in anyone who has investigated similar.

p.s. No metal fatigue or other metallurgical
failure, and they have only been up for a few months and hardly used.

Would need a lot more information before I could even sugggest begin to suggest all possibilities. Look through maintenance, operative training, installation procedures, non manufacturers parts, temperature and other similar factors first though.

One of the rather large sheds here had around 18 of these,and one did in fact come orf after a few days, subsequent investigation by this occifer deduced from about 20 paces the thing was completely 'skew wiff' and when a level was put on the rails they were some 15% out from true; I deduced further there was no fault of the manufacturer, it was a Friday afternoon fit (maybe Monday morning) it was the installers failing to follow the guidelines for said fitment.

Bet it 's the same with yours, put a level on the runners and see what you get.
