Please could anyone help me with my risk assessment on reversing a vehicle off a driveway, by helping me identify the risks and hazards.

For a suitable and sufficient RA I suggest you would need to provide more information regarding the reversing of vehicles from a driveway e.g. what type of vehicles? is the area adjacent to a public road/pavement etc.

That said, the issues you will have to consider include: do vehicles have reversing warning sound fitted, will there be a person assisting (banksman), what time of day/night, is it a busy area with people or cars etc.

As a matter of interest, why have you been tasked with completing a RA?

In that respect Daniel, you are attempting to assess bad practice. The Highway Code (as I was taught way back when) says reverse IN.
Lesser risks are posed on the driveway when reversing compared to the public road.
