keep things; short, sharp, to the point and simple with a sign on [opening the PTW] and a sign off [closing the PTW] area

Posted By Dave Merchant Don't wish to be rude, but "work at height" is a huge topic so you won't find a single PTW (even something you can adapt) that'll apply in every case. A PTW for using a portable ladder is not going to cover climbing a 1500ft transmission tower in a thunderstorm.
Are you using PFPE? If so do you have emergency and rescue equipment? Does your first aid provision cover those working at height? Are there issues of communication/lone working/audibility of alarms/COSHH/radio emissions/security/etc.? Are people placed at risk from falling objects? Are the places of work inspected before access? Is the access equipment (scaffolding/ladders etc) all inspected? Do workers require certification? Do your insurers care?

Posted By Ron Hunter I'm curious as to the circumstances of work at height which require you to have a formal permit system. I may need to be authorised to access the roof, and I will need risk assessment and most probably a method statement/SSoW, but what's the Permit for?
