Anybody direct me to a good site re attitude surveys?

Or have a basic copy of an employee &/or mamager survey proforma that they could pass across

The HSE site has links to their safety climate survey tool, which is inexpensive, useful and well respected.

I have sent you a template I once whipped up for surveys. A bit of tweaking and it may come of use.

It was set for a nebosh violence assignment but last sheet is general H&S culture type questions.

Not exactly scientifically proven etc but may come into use for data input and analysis once you have a suitable set of questions.

You will however have to create your own questionnaire as this is really a showing the results tool.

Hope it helps or at least gives ideas

Depending on the quality of information you need, in order to try to provide a worthwhile reply, I need to know answers to these questions

1. what do you wish to measure with an attitude survey

2. what will you and others do with replies

3. why do you wish to use a survey rather than other methods of data collection?
