Posted By Chris Mac Does anybody have any good links or documents they could provide for me which discusses near miss theory, and possibly Birds Pyramid. I would really appreciate it.

Posted By Pete48 Chris, try searching on "heinrich safety triangle" or "birds accident ratio triangle" or "heinriech/smith safety". It should bring up a wealth of results for you to follow. One I found http://www.safetycouncil.com/pdf/534Oakley.pdf Not sure what the original context of the article is but it does give some good pointers to the subject.
