I am planning introducing a safety performance league table each month to highlight to management which sites are rating high and so on. Would any body know where i could get examples or information regarding the aforementioned. Thanks

I would go for "KPIs" which are "SMART".
Before you roll your eyes...........go for a combination of inputs and outputs, positive and negative indicators eg
Accident rates
Number of near misses
Unsafe acts etc
and
Number of safety audits
Number of Safe acts
Training report
Safety actions done
Any other positives etc
Strongly advice, don't just go for accident data.

Thanks for that. my monthly site safety report rating is based on all safety criteria,i.e. accidents, documentation, therefore, just simply interested in a simple site safety percentage comparison table if available.

Be very careful we as a group have a league table and it provides false information.
The league tables have forced contract managers to under report or not report incidents at all, positive actions are completed by contracts just to get a 'good score' in the tables. The quality of some of the actions reported are really poor!

Trevor,
Why do you want the tables? As the last post points out it can lead to false reporting.
I would suggest taking a step back and look at what you want to prove/monitor without the drawback of the above. Do you want to provide support to the struggling sites? Bash them into shape? reward the well performing. All to often KPI/Performance Indicators/Leading&Lagging etc are used for the wrong reasons or interpreted incorrectly in their implementation so they eventally become meaningless.
Consider that if you want to compare sites, then dont have negative reporting systems (points based or otherwise) Incentive schemes are much discussed but used wisely can be useful. Is the points/league table linked to bonus?
What do your management want to know? Is it simply to bash the bad sites into shape?
Unless every site is identical, then can comparisons be made that are meaningful.
