Hi
I am trying to make up a simply card for employees to fill in for near Miss's / unsafe practises.
I want to try to encourage our employees to be more proactive is this matter.
And I wondered if anybody had any existing cards or forms that I may look at?
I am thinking of all cards being anonymous to encourage a no blame culture, and to try and get participation going, has anyone had success in this way.
All help appreciated

Had loads of success with our near miss(hazard report)reporting.
Just make sure you reply to all reports received in a prompt manner and keep a spreadsheet for data and trends.
When I receive a report I do a briefing at the start of shifts, I log each briefing.
For example last year I received 40 reports of broken pallets within the racking at our site.
I therefore implemented a stringent pallet checking regime that was formulated by the employees themselves and also liaised with suppliers with regard to the quality of pallets coming onto site.. The regime was re-briefed every time a similar report was received, so employee were made of the dangers time and time again.

Elliot, to make the card system work for you the employees may need guidance as to what is a "near miss" and also a reasonably good perception of hazards.
Otherwise you may be inundated with trivia which you will have to respond to, then if you don't, it falls flat. Otherwise it is a good way of establishing any questionable trends and good for communicating purposes.
a similar system works for me, but I had to train the employees first.
