Has any IOSH member formulated a safety culture survey they could share with me, comprising a set of questions that would be answered anonymously by all staff,managers and employees, in order to ascertain what differing perceptions (if any) groups of people have with regards to the safety culture of an business.
Just thought Id try and save myself the work of formulating one myself.

I seem to remember the HSE having a safety climate tool that is no longer available!

With a little bit of trial and error it is quite an interesting exercise to design your own bespoke questionnaire and tailored to your company. Normally a Likert 5-point scale questionnaire is used (multi-choice) with negatively and positively framed statements.

The first thing is to decide what characteristics of the culture you want to measure - domains. For example, incident reporting, risk taking behaviour, management, rules and procedures etc. About 5-6 questions in each domain will suffice, although some have a tendency to overlap.

I will be completing one shortly in my organisation, otherwise I would have sent you a sample questionnaire. Lost all my previous data when my hard drive collapsed.

Would any member be willing to share a safety culture survey they have formulated that I could adapt for my organisation. I would be particularly interested in hearing from anyone who has formulated a safety culture survey for Local Government.

Hi,
Apologies but may I also jump on the bandwagon. We are trying to implement an SMS at a German Airport and are having problems due to the language barrier of ascertaining an understanding of the level of safety awareness existing. I would be most grateful to have a copy of the questionnaire also.
