We have a reasonably good questionnaire that is posted out to potential contractors that covers H&S and Quality but I feel it lacks on the Environmental side.
So the question is-
Q. Does anyone have suggestions of questions for inclusion on the Enviro side?

Hi CFT
Yes we are 14001 and apart from asking in the questionnaire 'Are you 14001?' I'm looking for suggested questions that could be asked in relation to 14001 with a bit of lateral thinking for those companies that don't have or know about 14001 but would like to supply to / be a contractor to us.

Could you not re-adapt your initial contractor assessment pro-forma to include certain elements that would be relevant to the works they would be undertaking for you, rather than going with a 'scatter gun' approach in the hope the one cap fits all situations? Just a thought?
Clearly you won't want to work with an organisation that does not have a commitment to the environment, and as you have intimated, wish to either have a 'tagged on' approach or more direct, either way I might consider as examples the following questions:
1. Has your environmental system been assessed to BS EN ISO 14001? (include sections to detail Accreditation Body,Certificate of Registration Number etc. Or, do you have an EMS?
2. Do you have a documented Environmental Management System?
3. Do you have a person within your organisation responsible for Environmental Management?
4. Is your Environmental System audited? (if so, by whom and when)
5. Please enclose a copy of your Environmental policy
6. If you use sub contractors please state how you ensure SC compliance to your management systems.
7.Who has overall and final responsibility for Environmental matters?
8. Who is the most senior person in your organisation responsible for Environmental policy delivery on sites where your employees work?
9. How do you communicate changes of the policy to your employees?
10. Do you have a Company Safety Manual with relevant sections on health & safety, which describes in detail your site working practices, and if so, does the above manual include any references to procedures relating to environmental issues?
11. How do you ensure that the working practices and procedures used by your employees
on site are consistently in accordance with your Environmental Policy objectives and arrangements?
12. Have Managers and Supervisors who will plan, monitor, oversee, and carry out work on your contracts, received formal Environmental training?
13. What arrangements does your Company have to ensure new employees have knowledge of basic environmental issues?
14. How do you monitor the health of your employees? This is particularly important where exposure to noise, vibration, asbestos, hazardous materials and radiation is associated with your activities.
So Badger, the above questions are not meant to represent a definative list, but intended to offer some suggestions for you to integrate 'environmental' into your initial assessment and on-going active monitoring for contractors. As with all initial assessments they can on occasions appear overly complicated and time consuming; anyone without a EMS or QMS will perhaps have a completely different way of processing this, and those who do will understand the procedure that is required to retain the standard.
Have a play, it is only a few sample questions lifted from my own contractor assessment paper so there are no copyright restrictions on the wording.

It is not clear whether you are talking about constructrion contractors or others. If construction I would be asking
- Do you have formal procedures for the management of waste?
- What are the significant aspects of you work and the potential impacts?
- What procedures do you have for the management of hazardous waste?
etc. etc.
Yes ask the initial question as to whether they have a formalised EMS and if so provide an index and 3 previous audit reports. Then check for the things that are missing.
