I know its probably staring me in the face, BUT, is there any loss event legislation OR legislation that could be tied / linked / associated to it?

I don't understand. I think it may be the abrassive wheels regs that i gather you are seeking. This was changed, i believe because the regs at the time had something about all moving rotating parts had to be guarded, but as we all know a grinder has to have exposwd edges to works. Perhaps

There's also COMAH
The Management of Health and Safety at Work
Regulations 1999, regulation 5, requires
employers to plan, organise, control, monitor and review their health and safety arrangements.
Health and safety investigations form an essential part of this process.
I'm sure there's specific legislation relating to accidents on railways something like the Railways (Accident Reporting and Investigation) Regs.
