Posted By Roy McGraa I need a nudge in the right direction compiling a Risk Assessment and Method Statement for using a Fork Lift Truck in small unloading area within an industrial unit on an industrial estate. I have HSG6 to refer to but would appreciate a look at one or two comprehensive generic documents. I will, of course, be considering site specific issues such as uneven surfaces, blind corners, systems of work etc when compiling my own. Thank you

Part 1 FLT Driving Driving too fast(Hitting someone or something) Lifting a load that is too heavy(Tipping the truck, dropping the load) Driving on a slope(Tipping the truck/load) Driving with mast too high(Pedestrians below) Body parts being trapped in mast/other moving parts or outside of vehicle Poor maintenance of trucks(Failure of part causing accident) Other staff in the vicinity of working forklifts Awareness of trucks around corners or through curtains Reversing Forklift tipping Failure of chains or forks Part 2 General Operation Using forks to lift persons to height (prohibited) Unstable loads, Improper lifting / positioning of loads Overhead services Lift Trucks in the area – collision with fixtures etc Weather conditions Loading lorries High Loads Wide loads Gas bottles FLT Breakdowns, what to do Driver health - see below There's also a separate RA to use aa people cage. Other considerations: - ensure thorough examination by external competent person (usually via insurance company) - FLT Driver training - 3 stages - Basic Certificate, Specific job training, Familiarisation training. Keep records of training, accidents, disciplinary actions etc. - FLT Driver health - assessment needs to ask questions about heart condition, epilepsy, vision, hearing, mental health - Refresher training - ACoP says it should be given - Driver reassessment - when job or health changes etc John W
