Hello All,
Can anyone help me out with an example DSEAR 2002 assessment pro-forma (completed/blank).

Norman, I'm not sure what the purpose of your request is but I am assuming you are looking for inspiration to carry out your own risk assessment. If so then my advice is don't. If you are seeking advice to undertake a risk assessment for dangerous or explosive substances without having sufficient knowledge then you are probably placing yourself and others, including emergency service personnel, at risk. Of course I may be totally wide of the mark in which case I apologise.

There are numerous British Standards that can help you:
BS60079Pt10 which helps you calculate your hazardous areas and classification of flammable zones - 0,1, 2 etc. There is also a useful table in the back of this BS to help draw up the relevant details regarding your inventory holdings of HFLs and physical/chemical characteristics. This standard also covers guidance on assessing ventilation reliability/avaiability and adequacy.
Another good standard is BS1127 which covers the basic concept of explosion protection and is very good at helping you identify likely methods/sources of ignition - use this standard to help build a check list/prompts etc.
If dealing with liquids another good guide is the Institute of Petroleum guide Pt15, again for helping decide on flammable zone extents etc.
DSEAR is quite easy if you understand the basic idea of fire and vapour release potential. The hardest part is estimating the likelihood of leaks - there is data available regarding component leak frequencies (Frank Lees et al - good but very expensive text book). Important info if you are trying to demonstrate ALARP.
Unfortunately all the recognised publications don't come cheap. Neither does engineering experience.
Also remember the usual stuff about identifying training needs, control measures, display of 'EX' signs.
