I saw a video several years ago, which described the dissaster on the Piper Alpha oil and gas rig which happened some 20 years ago, and killed 167 people. The film was presented by one of the investigators, and I think was entitled Life and Death.
I would dearly love to see this film again, and perhaps buy a copy if it is stil available. Can anyone out there elp me locate it, or perhaps confirm the title and or give me any more information on it.

I Believe the film you are looking for is Seconds From Disaster by the BBC I'm not sure how you would get a copy but iv'e watched this many times and it is horrific but personally I find reading the Cullen Report even more so.

You can, but it is flv which installs its own toolbar.

Search 'FLV download youtube'

I am sure there are other, better programmes but this is quite easy to use.

Hi Barry, the best way to download from youtube is to use Zamzar, no installation required. Just copy and paste the page url into Zamzar.com/url/ and wait patiently. You will need to submit your e mail to get a message saying when the file is ready to download. So far this e mail address has not been abused in my experience. I have used it to download the Bradford fire clip

The latest Real Player allows you to download videos from all sources including youtube do it quickly before anyone mentions copyright!!!!

The one you're looking for is called "Life or Death" and is presented by Brian Appleton, one of the technical assessors who assisted Lord Cullen in the public enquiry into the disaster. I can't help you find a copy unfortunately, but agree that it is rivetting.

Try the "Step Change" website (http://stepchangeinsafety.net) as you may find a copy to borrow or purchase through the offshore safty community. You could also try putting a message out through the IOSH Offshore Specialist Group.

A very powerful video when I first used it shortly after it was first made available, I think through ICI back then. It left many members of the watching group in tears and no-one spoke for at least two minutes. I wonder whether it still has the same impact twenty odd years later? It should because the lessons are still as pertinent as ever, maybe even more so.

Two moments from the video that are implanted in my mind forever.
"Safety is not an intellectual exercise, it is a matter of life and death. The decisions we take affect the lives of our fellows and on the 6th of July we got it wrong."
and:
When under cross examination the CEO of ****** replied that the state of safety just before the accident was good. How did he know that he was asked. "because no-one told me otherwise" he replied.

I refer to the Brian Appleton video in my post, not the TV programmes. As Andrew says he was involved in the enquiry and is therefore able to speak with direct technical knowledge and authority and add his personal twist to the recording. I think I am correct in saying it was produced as a briefing for technical staff within ICI or perhaps Dupont but soon became available across the wider safety community of the time. The TV programmes whilst informative were, none the less, for a totally different audience.

Yes the video is very hard hitting and Brian Appleton is a very good speaker!
I have had a copy for some time but have not shown it as I was informed that a court order had been issued to prevent the video being shown as it mentioned non negligent companies in the text?
Does anyone know if this was/is the case???? Or if it is the case has the court order lapsed?

The video is called "life and Death!", and subtitled "Safety Management - Is that important?" which was produced by TVP Aberdeen (http://www.tvpfilm.com/) for the Safety Division Aberdeen Operations, BP Exploration. I cannot find a copyright date on the video box or even any copyright claim much to my surprise. Looking at the video which has a runtime of 50 minutes, it does have a BP copyright at the end.

I was hoping to find a DVD version of the video since our copy is getting decidedly tired - we like to give it to the middle and senior managers... sometimes it sinks home. The difficulty comes with it being a talking head, albeit a very good one which some people have difficulty in concentrating on for the length of it no matter how powerful it maybe.

I think it is so good that I would like to make it compulsory viewing for all middle and senior managements, especially of high risk workplaces.

It is likely that you would have to contact someone in BP to get a copy if one is still available.

Not sure what sector you work in but suggest that some of the videos which can be freely procured from U.S CSB are probably at least as relevant. Simple google for CSB will take you to their site.
