Posted By Safety officer I haven't got a completed one handy but consider:- Hazards - collision (with structure or pedestrian), overturning, drop load, mechanical failure, operator falls out etc Control measures - LOLER inspection, PUWER inspection, regular maintenance, pedestrian segregation, reversing siren, flashing light, head lights, training, one way system, seat belts, pre-use check sheet, warning signs, flooring in good condition, key removal when not in use. I'm sure there are more.
