Im looking for some guidance on a incident investigation process, i would like to put together a guidance check list to follow when carrying out the procedure.

Hi,
In addition to the HSE document mentioned above I've found Accident Management & Investigation by John Norton-Doyle published by Thomson-Gee to be very good.

Also don't forget the free IOSH Guide (available at 'information centre to the left), which is intended to be a little more advanced than the very basic HSE document.
