Posted By andy.c. After a couple of changes in personnel (1 non-voluntary)i have discovered that our records for FLT training are not available. When training is conducted the test sheets and authorisation to operate are presented to senior management, senior manager signs the authorisation and updates an electronic spreadsheet with the expiry date, the test sheets and authorisation are then filed, unfortunately it is the test sheets and authorisations that i say diplomatically are unavailable. I am confident that the training took place due to the electronic spreadsheet and have therefore re-issued authorisations signed with the original dates by management, also signed by trained person but dated when they signed. Whilst i am confident in the continued competence of the operators, and they have written authority if an enforcing officer asked to see a record of training i would not be able to provide evidence. Would an explanation of the missing documents as above be sufficient or would it be deemed reasonably practical to retest the affected operators which could mean over 100 test (as some operate 6 different machines). Any thoughts or pointers to similar situations most welcome, i am in 2 minds as to what to suggest to senior management but I'm keen that we do the right thing for the right reasons Andy

Posted By TonyB Hi Andy, What a pickle! It is unclear what training you are referring to. Is this the actual FLT training (i.e. the certificate) or the site specific requirement? The Authorisation to drive isn't an issue. However, I wouldn't have back dated it. All you have to do is be confident that each driver is competent at the time it was issued. The driver training however, may be an issue. I think your approach is valid, given the circumstances. Also, the problem will self correct after the drivers have undergone their next scheduled refresher training. I assume that this would within the next 3 years for all the drivers and earlier for most (Industry standard for refresher training is every 3 years). Hope it helps, TonyB

Hi Tony Thanks for the response, The authorisations are issued for three years and act as a trigger for the refresher training, i backdated them to ensure that they run concurrently and remain a cue for refreshers I thought this less confusing in the long run The paperwork that is missing is the 3 parts of basic training, Pre-shift check, theory paper, and practical skills test, you are quite right the situation will resolve itself within about 30 months with refresher training, but will i be OK without evidence of training if there is an issue surrounding one of the operators. Posted By Clare Gabriel Hmmm I think you really need to get your paperwork in order - a few ideas - do the drivers have their licences? You could copy those and they will have an expiry date? What about going back to the original trainer - they will surely have records of the courses they ran for you and be able to provide you with duplicate certs? Re the pre shift checks not much you can do there except ensure they are in place.

Posted By TonyB Hi Andy, Yet again Claire comments are exactly what I'd have said if she hadn't beat me to it! Who says (ex-)HSE Inspectors ain't consistent! Do what you can, and if asked by the HSE be open and honest, I'm sure they would understand. All the best, TonyB.
